Birth & Death Records

The Ottawa County Health Department can issue certified Birth Certificates for individuals born anywhere in Ohio from December 20, 1908 to the present date.

The health department maintains death certificates for those who died in Ottawa County starting December 20, 1908 to present date.

To Print off an Application for Birth or Death Certificates, please choose one of the following:

Application for Certified Birth Certificate

Application for Certified Death Certificate

When requesting a birth or death certificate by mail, please send the application form and a check or money order made payable to the Ottawa County Health Department, along with a self-addressed stamped envelope.

$22.00 per certified copy
